The Preparation Stage

Trying to navigate through situations that change your world as you know it can be very  difficult. Wouldn’t it be nice if you could refer to resources to assist you and your family? This  is only a small resource to help guide you with the planning for situations that may occur with  your loved one now and through their end. The first step would actually occur before any emergencies or diagnosis occurs that may not  end the way you plan for it. The first step is the preparation step. 

Preparation 

Even though it is very difficult, you need to have the discussion with your loved ones as to  exactly what their personal wishes or requests are for end of life decisions. You can print an  advance directive, also referred to as a living will, to help guide you with the questions you will  need to present to your loved one to obtain their personal preferences.

The website is: eforms.com and you have to select the state you reside in. Living wills do not have to be  prepared by an attorney but it must meet the state requirement. You do not have to pay to  have the living will prepared on this website, just print the blank copy for your state, and your  discussions can continue to answer all the questions.  

- A living will is not an actionable medical order, it is only used as a guide.

- Living will lasts forever until you change it. 

- If you are residing in New Jersey you can print one at: NJ.gov/health/advanceddirective.

- Once completed you need to be kept in a safe place until hospitalization occurs.  

- The second form that should be completed is called a POLST, which stands for Physician Orders for Life Sustaining Treatments. Each state has their own form. New Jersey’s form is  green. This form is different from your living will because you need to complete it with your  primary doctor, and it is actually doctor’s orders. This form will be honored and follow you  from your house, into the ambulance, and to the hospital. If long term placement is needed it  will follow you there as well.  

- POLST forms must be signed by 2 witnesses or be notarized, and are necessary if you are  seriously ill or at risk for a life threatening event. 

- You can print the form that is used in your state on: POLST.org
